- Download, print, and complete the Chain-of-Custody/Laboratory Test Request Form
- Ship or deliver your samples
along with a completed copy of the Chain-of-Custody/Laboratory Test Request Form to our laboratory

Please note: Samples cannot be analyzed without a completed Chain of Custody/Laboratory Test Request Form. To
avoid delays in the analysis of your samples be sure the Chain of Custody/Laboratory Test Request Form corresponds with your analysis. Chain of Custody/Laboratory Test Request Form MUST be signed by personnel relinquishing samples. Analysis
cannot be preformed without a signature.
